Complete the form and a scheduler will contact you within 1-2 business days to finalize your appointment. Your doctor’s order should be faxed to (317) 715-9990. An order is needed for all appointments except screening mammograms.

For X-rays, visit our Indianapolis/Carmel or Greenwood location during regular business hours and bring your order. X-Rays are handled as walk-ins only. Click for directions and office hours.
